The EloraTZP headline is striking: 23.3% average weight loss. But if I am trying to understand what a future treatment might mean in someone’s life, I also want to know how manageable it was.

What was tested?

Lilly’s 30 September announcement describes a 48-week Phase 2b trial in 367 adults with obesity or overweight and type 2 diabetes. EloraTZP combines eloralintide with tirzepatide, the active ingredient in Mounjaro and Zepbound. People were assigned by chance, with treatment kept unknown to participants and researchers. [1]

These are company-reported results presented at EASD, not a full peer-reviewed paper verified for this entry. The trial used separate injections; a combined formulation is planned for Phase 3. That is not an invitation to combine medicines yourself.

What does 23.3% describe?

The highest studied combination, eloralintide 9 mg plus tirzepatide 15 mg, produced average weight reduction of 23.3%, compared with 14.8% on tirzepatide 15 mg alone. The gap is 8.5 percentage points. HbA1c, a measure of blood sugar over time, fell by 2.9 and 2.4 percentage points respectively. [1]

Those doses identify study groups. The comparison with tirzepatide was an additional outcome; the primary comparison was with placebo. The figures estimate what would happen if people continued treatment, rather than promising the same result to everyone who starts.

How many people stopped?

Stomach and bowel adverse events were more common with the combination. Across its dose groups, 10.8%–27.0% stopped because of adverse events, compared with 2.9% taking tirzepatide alone and 16.7% taking placebo. [1]

The announcement does not match every stopping rate to a particular dose, so we should not attach 27.0% specifically to the highest-dose group. Describing symptoms as generally mild or moderate also does not erase the people who stopped.

The questions I still want answered

This report does not provide a useful women-specific or menopause analysis. It does not establish muscle preservation, relief from food noise or better everyday function.

I see promising human evidence, with a comparison against an active medicine, alongside important gaps. Fuller methods, safety results for each group and larger trials will help us understand the balance. Lilly’s plan to start Phase 3 in Q4 2026 is a development plan, not an approval. [1]
